You donot need a healthcare provider referral to schedule a new patient appointment.

Please print and fill out this HIPAA form if you need to add a relative or proxy.
If you had a sleep study from another location or have any other pertinent records that should be available for the provider at your appointment, please complete a Release of Medical Information form and send it to that facility or provider so that these records can be received in our office prior to your appointment. There is a copy of the form in the New Patient Packet.
Please arrive at least 15 minutes prior to your appointment if you have not already filled out the First Appointment Documents.

I had a sleep study done at Sleep Insights. It was a very nice experience for me. The technician was very professional, personable and made the procedure very relaxing for me. She explained everything to me step by step and was comforting especially during some anxiety that I was having. I look forward to my next visit for my results and would highly recommend Sleep Insights for anyone in need of a sleep study. The facility was beautiful as well. Very relaxing and homey.
As a RN myself, I am fully aware that it is becoming more and more difficult these days to come across quality patient care . . . Julia, NP and the team at Sleep Insights are shining examples of what it takes to deliver this kind of care. After suffering for years from life-altering, intense daily fatigue, I now not only have a diagnosis that they're working hard with to get me feeling better, but also hope. I am so hopeful for a brighter, happier future with my kids and family.
The facility is great, and the staff are as well. The sleep study itself, not the greatest. A real pain in the butt dealing with the probes and sensors and you really don't get a real good sleep. But hey, why complain when you're there for a reason right? My diagnosis was confirmed as being pretty severe and I am now wearing my mask religiously every night and sleeping like a baby. What more could you ask for?
