New Patient Resources
Interested in scheduling a new patient appointment? You can either call your preferred location to set up an appointment or contact us via email.
You do not need a healthcare provider referral to schedule a new patient appointment.
A new patient packet will need to be completed at your initial appointment, unless printed and completed ahead of time. If you don’t complete it before your visit, please plan on arriving 15 minutes prior to your appointment.

Please print and fill out this HIPAA form if you need to add a relative or proxy.
If you had a sleep study from another location or have any other pertinent records that should be available for the provider at your appointment, please complete a Release of Medical Information form and send it to that facility or provider so that these records can be received in our office prior to your appointment. There is a copy of the form in the New Patient Packet.
This HIPAA/Privacy Packet is for your reference–it is NOT necessary to print and sign.
Please arrive at least 15 minutes prior to your appointment if you have not already filled out the First Appointment Documents.
Please bring:
- Photo ID
- Insurance card(s)
- A list of medications you are currently taking.
- A method of payment for your visit. Co-pays, co-insurance or deductibles are due at the time of visit, prior to seeing the provider.
- If you have a CPAP machine, bring it to every appointment.
Please arrive at your appointment on time. Sleep Insights requires 24 hours’ notice for any cancellation; otherwise, you will be charged a $50 no-show fee for office visits and $100 no-show fee for sleep studies.
If you are later than 15 minutes for your appointment, we will have to reschedule your appointment.
INSURANCE
If you call your insurance company to confirm whether we’re a participating provider, please verify coverage for Sleep Insights Medical Associates, PLLC.
We do not accept Workers’ Compensation plans. We accept the majority of insurances, including:
- Most local commercial carriers (such as Excellus and MVP)
- Most national commercial carriers (such at Aetna and United Healthcare)
- Medicare
- Medicaid
We will contact your insurance to verify coverage and obtain prior authorization, if necessary. Additionally, for sleep studies and CPAP equipment/supplies, we will contact your insurance to determine the portion for which you will be responsible.
PAYMENT OPTIONS
If you don’t have health insurance, please ask our billing staff prior to scheduling your appointment for our private pay pricing and payment plan options.
We accept credit cards, debit cards with Visa/Mastercard logos, checks and cash. Payment can be made in person, on the Patient Portal or over the phone.
